Selenium

The trace mineral the thyroid concentrates more densely than any other tissue. Selenium runs the deiodinases that convert T4 into active T3 and the peroxidases that mop up the hydrogen peroxide hormone synthesis produces. Supplementation lowers thyroid antibodies reliably — whether that changes how patients feel is a separate and less settled question.

What Selenium Actually Does

Selenium is not a general-purpose antioxidant you take for vague protection. It works because roughly twenty-five human proteins are built with selenocysteine at their active site, and three of those — the iodothyronine deiodinases — are the enzymes that convert the T4 your thyroid secretes into the T3 your cells actually use.

The thyroid holds more selenium per gram than any other organ, and there is a reason. Making thyroid hormone requires hydrogen peroxide as a reagent. That peroxide has to be neutralised immediately, and the enzymes that do it are selenium-dependent glutathione peroxidases. A thyroid short on selenium is a gland generating oxidative damage as a by-product of its own normal function.

The Evidence

Autoimmune thyroiditis — antibodies fall. The foundational trial gave 200 mcg/day of sodium selenite to patients with autoimmune thyroiditis for three months and found a significant drop in thyroid peroxidase antibodies. A later systematic review and meta-analysis confirmed the effect across trials at three, six, and twelve months.

But the clinical question is unsettled. A 2024 systematic review and meta-analysis in Thyroid re-examined the randomised trials and reached a more measured conclusion: antibody titres respond, but evidence that supplementation changes the outcomes patients care about — symptoms, quality of life, levothyroxine requirement, progression to overt disease — remains limited. Falling antibodies are a laboratory result, not automatically a clinical one.

Graves' orbitopathy — the strongest result. A New England Journal of Medicine trial of 100 mcg twice daily for six months in mild Graves' orbitopathy improved quality of life, reduced eye involvement, and slowed disease progression. This remains selenium's most convincing indication.

Status determines benefit. The recurring theme across the clinical literature is that selenium helps people whose selenium status is low. Supplementing someone already replete adds risk without adding function.

Brazil nuts are not a dosing strategy. Selenium content varies by orders of magnitude with the soil the tree grew in — one nut may deliver 20 mcg or 300 mcg. They are an excellent food and a terrible measuring device.

Safety — The Window Is Narrow

Selenium has the shortest distance between "not enough" and "too much" of any common supplement. The gap between the 55 mcg daily requirement and the 400 mcg upper limit is roughly sevenfold, which sounds generous until you add a multivitamin, a thyroid formula, and a handful of Brazil nuts.

Early excess announces itself as garlic breath and a metallic taste. Chronic excess produces brittle nails and hair loss. And the epidemiology carries an uncomfortable signal: in populations that are already selenium-replete, long-term high-dose supplementation has been associated with increased type 2 diabetes risk. The dose-response curve is U-shaped, and the right-hand arm is real.

The practical rule: supplement to correct low status, not to accumulate. If you eat fish, eggs, and Brazil nuts regularly, you may already be at the top of the useful range.
