The Thyroid Optimization Protocol — Selenium + Iodine + Zinc + Tyrosine

A nutrient-status protocol for thyroid function, built on the two minerals the gland cannot work without. Selenium runs the T4-to-T3 conversion enzymes; iodine is the hormone substrate — and the one most likely to be overdone. Doses, timing, who should skip the iodine step entirely, and the labs to check before starting anything.

100 mcg. The cofactor for the deiodinases that convert T4 into active T3 and for the peroxidases that protect the gland from its own hydrogen peroxide. Trials in autoimmune thyroiditis used 200 mcg; 100 mcg is the maintenance dose. Total intake — diet included — must stay under 400 mcg/day.

150 mcg, and only if dietary iodine is genuinely absent (no iodised salt, dairy, eggs, or seafood). ⚠️ With Hashimoto's or nodular goitre, skip this step — supplemental iodine can precipitate hypothyroidism or hyperthyroidism. Never take iodine without adequate selenium on board.

Evening, with dinner

15 mg

15 mg. Supports deiodinase activity and receptor-level signalling. Take away from the selenium dose and away from calcium or iron. Above 40 mg/day chronically it induces copper deficiency — this is a maintenance dose, not a loading dose.

As needed, 30–60 minutes before demanding work

1–2 g, situational rather than daily. It will not raise thyroid hormone in a euthyroid person — it is here for the catecholamine side of fatigue, not the hormonal side. ⚠️ Contraindicated in hyperthyroidism; separate from levothyroxine by four hours.

Protocol Overview

This protocol has a narrow remit, and stating it plainly is the point: it corrects nutrient inputs to a normally functioning thyroid. It does not treat thyroid disease. Hypothyroidism is treated with levothyroxine, not with minerals, and no combination of supplements substitutes for that.

What nutrient status does influence is the machinery around hormone production:

  • Selenium activates the deiodinases that convert T4 into the T3 your cells use, and the peroxidases that neutralise the hydrogen peroxide hormone synthesis generates.
  • Iodine is the substrate the hormone is literally built from — essential, and the single easiest way to make a thyroid worse if overdone.
  • Zinc supports deiodinase activity and receptor-level signalling.
  • L-tyrosine supplies the amino acid backbone — almost never the limiting factor, and included here honestly as the weakest link in the stack.

Selenium is the step with the strongest data. Everything else is conditional.

Before You Start: Get the Labs

Supplementing a thyroid you have not measured is guesswork. The minimum panel:

  1. TSH — the screening test. Interpret it against age-adjusted ranges rather than a single cut-off.
  2. Free T4 — distinguishes subclinical from overt hypothyroidism.
  3. TPO antibodies — the single most informative test here. Positive antibodies mean Hashimoto's, and Hashimoto's changes the iodine decision completely.
  4. Ferritin and vitamin D — commonly low alongside thyroid complaints, and commonly the actual cause of the fatigue being blamed on the thyroid.

If TSH is elevated with positive antibodies, this protocol is a supporting layer for a conversation with an endocrinologist, not a replacement for it. The evidence on when treatment helps is covered in the subclinical hypothyroidism review.

How to Build It

  1. Weeks 1–4: selenium alone at 100 mcg. One variable at a time. If antibodies are positive, 200 mcg is the dose the trials used — with the 400 mcg ceiling counted across all sources.
  2. Decide on iodine deliberately. No iodised salt, no dairy, no eggs, no seafood → add 150 mcg. Hashimoto's or nodular goitre → do not add it. Pregnant or planning pregnancy → requirements rise, and this belongs with your obstetric care, not a supplement stack.
  3. Add zinc at 15 mg in the evening if intake is low. Keep it away from the selenium dose.
  4. Use tyrosine situationally, for cognitively demanding work under fatigue — not as a daily thyroid supplement.
  5. Retest TSH, free T4, and TPO antibodies at 12 weeks. Antibody titres are the marker most likely to move. Note that falling antibodies do not automatically mean you feel better — the 2024 meta-analysis is explicit that patient-relevant outcomes remain unproven.

Safety & Notes

  • Selenium's window is narrow. The requirement is 55 mcg and the upper limit is 400 mcg. Add up your multivitamin, your thyroid blend, and your Brazil nuts before adding a standalone dose.
  • Iodine is the step that hurts people. High-dose protocols (12.5 mg, 50 mg, Lugol's, kelp) are 100–300× the requirement and carry real risk of iodine-induced hypothyroidism or hyperthyroidism.
  • Zinc above 40 mg/day chronically induces copper deficiency. Fifteen milligrams needs no copper counterbalance.
  • Tyrosine is contraindicated in hyperthyroidism and with MAO inhibitors.
  • Levothyroxine spacing: take it fasted, and keep minerals — especially calcium, iron, and zinc — at least four hours away, or you will reduce absorption of the one thing in this list that actually treats disease.
  • Pregnancy changes every recommendation here. Iodine requirements rise, selenium safety data are thinner, and the stakes are higher. That is a clinical conversation.

Foundations First

Thyroid symptoms are famously non-specific, and the honest observation is that most people who feel cold, tired, and foggy do not have a thyroid problem. Sleep debt, chronic energy restriction, overtraining, iron deficiency, and low vitamin D produce the same complaint list and are far more common. Persistent low-calorie dieting in particular lowers T3 — and the fix is food, not selenium.

Rule out the ordinary before optimising the exotic.
