Curcumin

The most-studied polyphenol in supplements and the one where the label tells you least. Plain curcumin is barely absorbed — the difference between formulations spans an order of magnitude, which means the milligram figure on the bottle is close to meaningless without knowing the delivery system behind it.

The Absorption Problem

Turmeric root is roughly 2–5% curcuminoids, and curcumin itself is poorly soluble in water, unstable at intestinal pH, and conjugated so aggressively by the gut wall and liver that plain extract produces barely detectable plasma levels. Every serious question about curcumin runs into that fact first.

Two engineering answers exist, and they are not equivalent.

Piperine. The classic human study found that 20 mg of piperine alongside 2 g of curcumin raised bioavailability by around 2000%. It works by inhibiting the glucuronidation that clears curcumin — which is effective and also the source of its main drawback: piperine inhibits the same enzymes that metabolise many prescription drugs.

Formulation. Phytosome (curcumin-phosphatidylcholine), micellar, and colloidal preparations improve absorption without touching drug metabolism. A comparative human study of curcumin formulations found differences spanning more than an order of magnitude between products at equal curcuminoid content.

The practical consequence: "1000 mg of turmeric extract" tells you nothing useful. A gram of standard 95% extract and a gram of phytosome curcumin are different drugs at the level of what reaches your bloodstream.

What the Evidence Supports

Systemic inflammation. An updated meta-analysis of randomised controlled trials found curcumin lowers C-reactive protein. This is the cleanest quantitative signal in the literature — a real, measurable effect on an objective marker.

Osteoarthritis. A systematic review and meta-analysis of turmeric extracts and curcumin in joint arthritis found symptom relief, with several trials reporting effects comparable to standard analgesics. The trials are mostly small and short, so the appropriate confidence is "promising and reasonably consistent" rather than "established".

Combination therapy. A network meta-analysis of knee osteoarthritis compared Curcuma longa, Boswellia serrata, and mixed formulations directly — supporting the standard pairing rather than either extract alone.

Where enthusiasm outruns data: cancer prevention, Alzheimer's, and depression. Mechanistic plausibility is abundant; human outcome evidence is not.

Safety — Better Than Its Reputation, Not Flawless

Curcumin is well tolerated at these doses and turmeric has a long culinary safety record. Three cautions are worth taking seriously.

Liver injury. A recent comprehensive review by the US Pharmacopeia examined reported hepatotoxicity cases from turmeric and curcuminoid supplements and characterised them as rare — but they are real, and reports cluster around high-bioavailability and piperine-enhanced products. Culinary turmeric is not the issue; concentrated, absorption-enhanced supplements are. Stop and get liver enzymes checked if jaundice, dark urine, or unexplained fatigue appear.

Drug interactions. Piperine's inhibition of CYP3A4 and glucuronidation is a genuine mechanism for raising the levels of co-administered drugs. If you take anything with a narrow therapeutic index, choose a phytosome product instead.

Bleeding and bile. Curcumin has antiplatelet activity — discontinue two weeks before surgery and be cautious with anticoagulants. It also stimulates gallbladder contraction, which makes gallstones and biliary obstruction genuine contraindications rather than boilerplate.
