The oldest liver herb in the cabinet, and the one with the most trials — mostly small ones. Silymarin has consistent signal on liver enzymes and metabolic markers in fatty liver disease, poor oral bioavailability that the good formulations fix, and an unusually clean safety record.

What Is Milk Thistle?
Milk thistle (Silybum marianum) has been used for liver complaints for centuries. The active fraction is silymarin, a complex of flavonolignans dominated by silybin (also called silibinin), which does most of the pharmacological work.
It is the rare traditional herb with a real trial literature behind it. It is also the rare supplement whose central problem is not efficacy but absorption.
The Bioavailability Problem
Plain silymarin is poorly water-soluble and poorly absorbed. Pharmacokinetic work on the silybin–phosphatidylcholine complex showed a large improvement in bioavailability over the unformulated extract — which is why "milk thistle 1000 mg" on a cheap label and a standardised phytosome extract are not comparable products at all. If you are going to take this, the formulation is the decision that matters more than the milligram count.
What the Evidence Shows
Fatty liver disease. A systematic review and meta-analysis of silymarin in NAFLD/NASH found improvement in liver enzymes and related markers. The effect is real but modest, and the underlying trials are mostly small and short.
Insulin resistance. A separate meta-analysis of randomised trials found silymarin improved insulin resistance and sensitivity measures. This matters because metabolic dysfunction is now understood to drive most fatty liver disease — the two findings are the same story told twice.
What it is not. Milk thistle does not reverse cirrhosis, does not license heavier drinking, and is not a substitute for treating the cause of liver injury. Intravenous silibinin's role in Amanita mushroom poisoning is a clinical use that has almost nothing to do with a capsule taken daily.

Safety
Silymarin has one of the cleanest safety records in the botanical category. An updated safety review found adverse events at close to placebo rates, with mild gastrointestinal upset the usual complaint. Two genuine cautions: people allergic to the Asteraceae family (ragweed, daisies, chrysanthemums) can react to it, and silymarin weakly affects some drug-metabolising enzymes and transporters — rarely a clinical problem, but worth checking if you take a narrow-therapeutic-index medication.
